Looks like an applied digital filter. If you look closely, everything in a straight line across the middle of the photo is in focus. If it was an optical effect, the focus would change based on the distance from the photographer.

Yeah, simple little filter with some adjustments, all in-phone via a tilt-shift app.

I can spend hours with layer masks and filters in PS, painting in extremely convincing shallow depth of fields to really give the tilt-shift effect some convincing pop. The dead give away in a quick and dirty TS is always gonna be the straight line where the focus is sharp, regardless of the position of objects (a street light in the focused area getting blurry with the rest of blurry background kills the illusion, or as Mistermarc pointed out here, the telltale signs are evident in this little toy camera ferry shot).

Wasn't trying for a tilt-shift effect, but wanted to draw the viewers attention to the house in an otherwise flat and blah pic, by making elements around it, behind it, kinda subtly lost in bokeh. Maybe it's not so subtle. Not my best work by any means, but one of those things where you get to a point in fidgeting with it and shrug, "good enough", and move on.

I love those quick iPhone apps. Maybe it's just more fun when you don't take it as seriously. I use the tilt-shift filter in instagram all the time.

Actually; it just so happens that all my best pictures are taken when I trip forward.  No pain, no gain right?
