No government oversight or accountability, extremely varied treatment plans with many fantastical, no scientific rigor involved in treatment plans, can be incredibly expensive, deaths in programs, and claims of success  rate have no data supporting them.

And there is a "bounty Hunter" industry that goes out and nabs addicts with insurance that can pay.

Taboo social issues have always brought the lowest types of scum oozing out from under the woodwork. That's why these clinics must be heavily regulated, and should probably not even be allowed to operate as commercial enterprises.
